Policy on Participant Liability Release
1. POLICY
Participants in the Leadership Corvallis program are required to sign the “Leadership Corvallis Waiver and Release of Liability form”:
/file_download/26/005a-Waiver-and-Release-of-Liabilty.pdf by the first day of the program. Participants who choose not to sign the form will not be allowed to continue in the program.
The Waiver and Release of Liability form protects Leadership Corvallis in the event of an accident and also serves as a model release form for the use of photos and video to publicize the program.
2. PROCEDURES
2.1 – The Program Host will collect the signed and dated Waiver and Release of Liability forms from class participants on the first day and will ensure that all have signed the form.
2.2 – The signed Waiver and Release of Liability forms will be maintained with the class years’ roster and attendance records and will be maintained in accordance with applicable Federal, State and Local statutes.
